Disability Forms

Disability reports can be prepared in our business office. You should anticipate at least a 1-week turn-around time for completion. There will be a $5.00 charge for each disability form completed, to be paid at the time the form is presented.

Surgical Scheduling

If you find you will be needing surgery, please keep the following in mind. A large number of insurance companies now require prior authorization or a second opinion prior to scheduling surgery. It is important, therefore, that we have accurate insurance information from you, including policy numbers, addresses and phone numbers.

Casts

Casts always seem to be too long or too heavy, but should never be too tight or uncomfortable. Don't hesitate to call our office should you be concerned about your cast.
